Revocations, on the other hand, are commonly reversed: a carrier whose authority is revoked for an insurance lapse can file a fresh BMC-91 form, pay any outstanding fees, and have the authority reinstated, often within 21 days. The reinstatement appears as a separate row on the day FMCSA serves the discontinued-revocation order.
