Carrier Insurance-Lapse Early Warning

Motor carriers showing signs of BMC-91 instability over the last 30 to 90 days — derived from public FMCSA Licensing & Insurance filings. This is a leading indicator, not a substitute for live verification.

All tiers · U.S. carriers

Every authorized motor carrier in the United States is required to keep an active BMC-91 or BMC-91X public-liability filing on record with the FMCSA. When that filing is cancelled and a replacement is not promptly observed, the carrier is on a 30-day clock toward automatic authority revocation. This page surfaces carriers in that window — and carriers that have churned through three or more insurance events in the last twelve months — so brokers, shippers, and other carriers can ask the right questions before tendering a load.

Honest disclaimer: a flag on this page is a derived signal, not legal advice. Cancellations followed by replacements within 30 days are normal carrier behavior. Use the flag to escalate due diligence, not to make an automatic disqualification decision. Always verify a carrier's current insurance status directly on FMCSA L&I before acting on this information.

How we classify these carriers Auditable predicates · live data source

Every tier on this page is derived from a deterministic predicate against the public FMCSA Licensing & Insurance dataset. The exact rule is listed below so brokers, shippers, and underwriters can verify any classification directly against the source records.

Critical risk

A BMC-91 / BMC-91X cancellation has been observed in the last 30 days AND no replacement filing has been received.

Audit predicate
recent_cancel_date >= today − 30 days AND (replacement_date IS NULL OR replacement_date < recent_cancel_date)

FMCSA's revocation clock starts at the cancellation date. A carrier in this state typically has days, not weeks, before operating authority is administratively revoked unless a new BMC-91 is on file.

Elevated risk

Either three or more insurance events (cancellations, replacements, reinstatements) in the last 12 months, OR a 30–90 day BMC-91 cancellation that has been replaced.

Audit predicate
event_count_12mo >= 3 OR (recent_cancel_date >= today − 90 days AND replacement_date >= recent_cancel_date)

A high event count signals an unstable insurer relationship that historically precedes authority-revocation cycles. A 30–90 day replacement is itself benign, but combined with no public A.M. Best rating on the replacement underwriter, the coverage quality is unverified.

Moderate risk

Currently rolled up into the Elevated tier. The spec's original Medium tier required A.M. Best ratings on replacement underwriters; we have not yet integrated that source, so Medium-pattern carriers surface as Elevated until the rating data lands.

Audit predicate
(reserved — collapsed into HIGH; no separate predicate is evaluated today)

Listed here for transparency so users know we are NOT silently downgrading any Medium-pattern carriers. They appear in the Elevated tier and will be split out once A.M. Best ratings are integrated.

Source & freshness: Computed from public FMCSA Licensing & Insurance (L&I) records — InsurancePolicy and InsuranceEvent tables, refreshed via the daily 04:00 UTC import cron. The materialized view `mv_insurance_watch_classified` re-runs the classification nightly; per-carrier pages re-run it on request. Sources Freshness Methodology Disclaimer

Frequently asked questions

What is a BMC-91 filing? ▾
BMC-91 (and BMC-91X) is the FMCSA form an authorized motor carrier files to prove it carries the federally-required public liability insurance — at least $750,000 of bodily-injury and property-damage coverage for general freight, $1 million or $5 million for hazardous-materials carriers. Without an active BMC-91 filing on record, the carrier's operating authority is automatically revoked.
Why does an insurance cancellation matter before authority is revoked? ▾
FMCSA's revocation process runs on a 30-day clock from cancellation. During that 30-day window the carrier is still legally authorized but is operating under a known BMC-91 compliance defect. A CRITICAL watch tier is not the same as confirmed absence of BIPD/Primary on file — check the carrier's active liability filings before treating the load as uncovered. This page surfaces the filing gap so brokers and shippers can verify coverage before a load moves.
What should brokers do with this information? ▾
Treat it as one input alongside the carrier's safety rating, inspection history, and authority status. Before tendering a load to a flagged carrier, request a current certificate of insurance directly from the producer (not the carrier) and verify on FMCSA L&I that a replacement BMC-91 has been accepted. Many flagged carriers will resolve the gap routinely — this is a flag, not a verdict.
How is this different from authority revocation? ▾
Authority revocation is the FMCSA's official action — it happens after the cancellation has been on file for the regulatory window and no replacement has been received. The insurance-watch flag is the leading indicator that fires the day the cancellation lands, before any official action. Once authority is actually revoked, the carrier appears on the standard authority-status pages, not here.
How often is this updated? ▾
Whenever the FMCSA Licensing & Insurance import pipeline runs (typically daily), the underlying InsurancePolicy and InsuranceEvent tables are refreshed and this page recomputes the tier classifications on next request. The page header shows the most recent as-of date observed in our data, and a freshness warning appears if that date is more than seven days old.
Where does the data come from? ▾
Every filing shown here is sourced directly from FMCSA's public Licensing & Insurance (L&I) system — the same underlying records the agency itself uses to administer operating-authority status. We add only the derived classification (Critical / Elevated / Moderate); the underlying events are public-domain and searchable on FMCSA L&I.
