TL*THE CONTACT OWNS A 1995 FORD F350. THE VEHICLE HAS UNIROYAL LAREDO LTL, SIZE LT235/85R16 TIRES. WHILE DRIVING AT AN UNKNOWN SPEED, THE FRONT DRIVER SIDE TIRE TREAD SEPARATED AND CAUSED THE VEHICLE TO VEER OFF THE ROAD, DOWN AN EMBANKMENT, AND INTO SOME TREES. THE VEHICLE CRASHED AND FLIPPED OVER ON THE DRIVER SIDE. THERE WERE TWO OCCUPANTS IN THE VEHICLE. THE CONTACT'S WIFE WAS SEATED IN THE PASSENGER SEAT AND WAS FATALLY INJURED. THE CONTACT RECEIVED MINOR INJURIES. A POLICE REPORT WAS FILED. AN INVESTIGATOR PROVIDED THE COMPLAINT INFORMATION ON THE CONTACT'S BEHALF. THE PURCHASE DATE AND DEALER INFORMATION WERE UNKNOWN. THE CURRENT AND FAILURE MILEAGES WERE 124,055.
