BACK OVER PREVENTION

Owner Narrative

The contact owns a 2016 Ford Fusion. The contact stated that while driving at an undisclosed speed, the message "No Coolant" was displayed on the instrument panel, and there was smoke coming from outside of the vehicle. The vehicle was taken to an independent mechanic, where it was diagnosed with a blown head gasket. The contact was informed that the engine needed to be replaced. The independent mechanic made the contact aware of an undisclosed recall; however, the VIN was not included in a recall related to the failure. The vehicle was repaired. The manufacturer was notified of the failure. The contact stated that while reversing at an undisclosed speed, the rearview camera image was inverted and the rear passenger’s side of the vehicle crashed into an unoccupied vehicle. No warning lights were illuminated. The failure had occurred on several occasions. A police report was not filed. The contact received notification of NHTSA Campaign Number: 25V695000 (Back Over Prevention); however, the part to do the recall repair was not yet available. A local dealer was not contacted. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was made aware of the issue. The failure mileage was approximately 99,000.
